The Transition Coalition offers "a self-assessment instrument designed to help schools and districts determine the most critical needs within a transition program. This instrument is best utilized if multiple transition stakeholders complet...e it (e.g., teachers, administrators, parents, others). The data can be then compiled into a single summary for the district." The tools is free - someone from the district has to create a user profile (the link is on the left-hand side of the homepage under "Quick Links."). I think all districts should go through this exercise.
